Smile for a Lifetime Foundation is a charitable non-profit organization
that provides orthodontic
care to individuals who may not have the opportunity to acquire
assistance.
Launched in 2008, Smile for a Lifetime Foundation aims to reach
individuals with financial challenges, special situations, and orthodontic
needs. The Foundation sponsors the orthodontic care of hundreds
of patients each year.
Smile for a Lifetime Foundation has participating orthodontists
throughout the US. Each chapter has its own local Board of Directors
who choose patients treated by the Foundation.
Who Qualifies:
To become a candidate for an orthodontic scholarship, interested
patients must complete or have completed for them, a Smile for a
Lifetime Foundation application that is available to download from
this page.
Applicants must be children ages 10-17, residing in Novato or San
Rafael, California, with a household income that does not exceed
200% of the US Department of Health and Human Services' poverty

Our mission at Smile for a Lifetime is to create self confidence,
inspire hope, and change the lives of children in our community
with the gift of a smile.
Our Doctor:
Dr. Don Wilson is the founder of the Novato/San Rafael Chapter
of Smile for a Lifetime Foundation. He is an orthodontic specialist

leadership and passion for community improvements are characteristics

Deadline to submit applications for 2012 orthodontic scholarships
is June 13th, 2012
Letters of recommendation are mandatory and should be inspiring
character references explaining the unique reason why this applicant
should be chosen. These can be written by a neighbor, dentist,
teacher, friend or other community member. Please do not submit
more than two letters, and limit each reference letter to one
page each. Please type or print clearly with black ink (no pencil).
Photos are mandatory. See application for details. Pictures
of the applicant's teeth must by clear.
If selected for the next phase of screening, proof of income
and proof of residence in Novato or San Rafael will be required.
Your application, letters of recommendation, pictures, and
any other documents provided during the application process, will
not be returned to you and will become property of Smile for a
Lifetime foundation.
If chosen for an orthodontic scholarship, applicant must agree
to only morning appointments at Dr. Don Wilson's office.
Applications that do not meet the above criteria will not be
voted on by our Board of Directors.
