It is our desire to provide for your orthodontic needs both thoroughly and efficiently. We would like you to allow an hour and a half for the appointment. As part of the complimentary consultation, we will take an x-ray, photos, and Dr. Don will do an extensive exam. He will review a clinical diagnosis and discuss a treatment plan along with the procedures involved.

We will also discuss treatment time, cost and the types of braces available. Then, if Dr. Don feels that you are ready, we can go ahead and get started with impressions.

In order to provide a more detailed and informative examination, we ask that you complete our patient information and health history forms. Click on the link above to download the forms. Please print all pages and bring the completed forms to your appointment.
